A Clinical Pharmacist position focused on sterile compounding is available in Dallas, TX, for a 13-week contract assignment. The schedule alternates between two shifts: one day from 7:00 AM to 3:30 PM and the next from 12:00 PM to 8:00 PM, offering a balanced work routine.
Key Responsibilities:
- Assess patients for home infusion therapy and determine their eligibility for home-based care.
- Develop and adjust personalized medication therapy plans guided by clinical assessments, lab values, and pharmacokinetic data.
- Prepare, verify, and dispense sterile medications ensuring compliance with USP standards, federal and state regulations, and internal pharmacy policies.
- Educate patients, caregivers, and healthcare providers on medication regimens, infusion administration, and safe medication use practices.
- Collaborate with healthcare teams to maintain continuity of care and monitor patient outcomes.
- Manage infusion pumps and medication administration devices while upholding quality assurance and safety protocols.
- Provide comprehensive drug information, maintain meticulous documentation, and support adherence to regulatory and medication safety standards.
Qualifications and Experience:
- Doctor of Pharmacy (PharmD) degree or graduation from an accredited School of Pharmacy.
- Active, unrestricted Pharmacist license in Texas.
- One or more of the following:
- Minimum one year of sterile IV compounding experience.
- Experience with non-sterile compounding, immunizations, and Medication Therapy Management (MTM).
- Completion of an ASHP-accredited Pharmacy Residency.
- Completion of an Advanced Pharmacy Practice Experience (APPE).
- Equivalent pharmacy experience.
- Strong clinical knowledge of home infusion pharmacy practices and ability to interpret laboratory results applying pharmacokinetic principles.
- Excellent communication and patient counseling skills with a commitment to outstanding customer service.
