Clinical Pharmacist | Dallas, Texas

A clinical pharmacy role is available for a Sterile Compounding Pharmacist on a 13-week contract in Dallas, TX. The role features a rotating schedule with one week on the 7:00 AM to 3:30 PM shift followed by one week on the 12:00 PM to 8:00 PM shift.

Key Responsibilities:

• Evaluate patients for eligibility and appropriateness of home infusion therapy.
• Develop, monitor, and adjust individualized medication regimens using clinical assessments, lab results, and pharmacokinetic data.
• Prepare, verify, and dispense sterile medications adhering to USP standards and regulatory requirements.
• Provide education to patients, caregivers, and healthcare providers on medication therapies and infusion techniques.
• Collaborate closely with healthcare teams to ensure continuity of care and monitor patient outcomes.
• Manage infusion pumps and related equipment with focus on quality assurance and safety.
• Maintain accurate documentation, support regulatory compliance, and promote medication safety initiatives.

Qualifications & Desired Experience:

• Doctor of Pharmacy (PharmD) or graduate from an accredited School of Pharmacy.
• Active, unrestricted Texas Pharmacist license.
• At least one of the following: • Minimum 1 year of sterile IV compounding experience, • Experience with non-sterile compounding, immunizations, and Medication Therapy Management (MTM), • Completion of an ASHP-accredited Pharmacy Residency, • Completion of an Advanced Pharmacy Practice Experience (APPE), • Equivalent pharmacy experience.
• Strong clinical knowledge of home infusion pharmacy practices.
• Expertise in interpreting laboratory values and applying pharmacokinetic principles.
• Excellent communication and patient counseling skills.
