This position focuses on delivering clinical pharmacy services in a Long-Term Acute Care (LTAC) setting within Marin County, CA. It offers a flexible part-time schedule with the potential to transition into a full-time role, allowing pharmacists to engage in a hospital-like environment while working closely with medically complex patients in a smaller, patient-centered facility.
Key Responsibilities:
- Review and verify medication orders specific to LTAC patients
- Provide clinical pharmacy support including pharmacokinetics and anticoagulation monitoring when applicable
- Assist with medication dispensing workflows and automation systems, such as Pyxis
- Collaborate effectively with interdisciplinary care teams to optimize medication therapy and patient outcomes
- Ensure safe, accurate, and effective medication management for long-term, medically complex patients
Qualifications:
- Bachelor of Science or Doctor of Pharmacy (PharmD) from an accredited institution
- Active, unrestricted California Pharmacist License in good standing
Preferred Experience:
- 2–3 years in retail pharmacy with a strong interest in transitioning to a clinical setting
- Prior inpatient or hospital pharmacy experience beneficial
- Familiarity with TPN preparation/monitoring
- Knowledge of pharmacokinetics and anticoagulation management preferred
- Current BLS and ACLS certifications a plus
