Clinical Pharmacist in Dover, DE

Deliver comprehensive ambulatory clinical pharmacy services as an integral member of a healthcare team, optimizing medication therapy and enhancing patient care under Collaborative Practice Agreements (CPAs). This role centers on managing therapeutic plans through evidence-based medication use, within an ambulatory network dedicated to Comprehensive Medication Management (CMM).

This high-visibility clinical leadership position involves serving as a resource for providers, staff, and patients, supporting chronic disease management, specialty medication coordination, and pharmaceutical care excellence.

Key Responsibilities:

• Provide ambulatory clinical pharmacy services under pharmacy CPA, including assessment, monitoring, and documentation of therapeutic plans.
• Conduct patient medication management visits encompassing medication reconciliation, lab monitoring, counseling, and disease state management.
• Collaborate with healthcare providers and care teams to implement evidence-based medication interventions promoting quality, safety, and cost-effectiveness.
• Manage referrals with closed-loop communication to referring clinicians.
• Facilitate patient access to medications via prior authorization, financial assistance, and benefit investigations.
• Oversee specialty medication therapy initiation, monitoring, and continuity.
• Offer drug information and education to patients, families, and healthcare teams.
• Precept pharmacy students, interns, and residents as assigned.
• Develop and enhance systems and services to improve patient care.
• Engage in research, publications, committee participation, policy development, and quality improvement initiatives.
• Document all clinical interventions and report outcomes in the electronic health record according to established standards.

Required Qualifications:

• Active Delaware Pharmacist License in good standing.
• Board of Pharmacy Specialties (BPS) Certification in Pharmacotherapy, Ambulatory Care, Cardiology, or Geriatrics (mandatory).
• Minimum one year of clinical pharmacy experience, including residency.
• Bachelor of Science in Pharmacy or Doctor of Pharmacy degree.

Preferred Qualifications:

• Doctor of Pharmacy (PharmD) strongly preferred.
• Completion of PGY1 or PGY2 ambulatory care residency highly desirable.
• Experience with Collaborative Practice Agreements, specialty medication management, CMM, chronic disease management, or population health pharmacy.
• Prior authorization and patient financial assistance experience.
• Preceptor experience is a plus.
