Clinical Pharmacist in Dover, DE

This role offers an opportunity to deliver advanced ambulatory clinical pharmacy services as an integral member of a dynamic healthcare team. As a credentialed pharmacist practicing under Collaborative Practice Agreements (CPAs), you will optimize therapeutic outcomes, improve medication access, and contribute to cost-effective care delivery. This position emphasizes a nationally recognized Comprehensive Medication Management (CMM) model, shaping the foundation of a leading ambulatory pharmacy program.

Key Responsibilities:

• Provide clinical ambulatory pharmacy services through CPA credentialing, including assessment, monitoring, and documentation of therapeutic plans.
• Conduct patient medication management visits involving reconciliation, laboratory monitoring, counseling, and chronic disease management.
• Collaborate with multidisciplinary providers and teams to implement evidence-based medication interventions aimed at enhancing quality, safety, and reducing costs.
• Manage provider referrals with closed-loop communication to ensure coordinated patient care.
• Facilitate patient access to medications by coordinating prior authorizations, financial assistance, and benefit investigations.
• Oversee specialty medication therapies, ensuring safe initiation, monitoring, and continuity.
• Deliver drug information and education to patients, families, and healthcare professionals.
• Precept pharmacy students, interns, and residents as assigned.
• Develop and enhance pharmacy systems and services to elevate patient care.
• Participate in research, policy development, quality improvement initiatives, and committee work advancing ambulatory pharmacy services.
• Document interventions and report outcomes in the electronic health record in line with national ambulatory pharmacy standards.

Qualifications:

• Active Delaware Pharmacist License in good standing.
• Board of Pharmacy Specialties (BPS) Certification in Pharmacotherapy, Ambulatory Care, Cardiology, or Geriatrics (required).
• Minimum one year of clinical pharmacy experience, including residency.
• Bachelor of Science in Pharmacy or Doctor of Pharmacy (PharmD strongly preferred).
• Preferred: PGY1 or PGY2 ambulatory care residency, experience with Collaborative Practice Agreements, specialty medication management, CMM, and chronic disease management.
