A closed-door Long Term Care pharmacy is expanding operations and seeking dedicated IV pharmacists to join their team in the Dublin, OH area. This direct hire opportunity includes two positions: one IV Lead Pharmacist on the first shift and one IV Staff Pharmacist on the second shift. Both roles require an Ohio pharmacy license.
Location: Dublin, OH (Columbus area)
Schedule:
- IV Lead Pharmacist: Monday – Friday, 7:00 AM – 3:30 PM or 8:00 AM – 4:30 PM (flexible)
- IV Staff Pharmacist: Monday – Friday, 4:30 PM – 12:00 AM or 6:00 PM – 2:30 AM (flexible)
Key Responsibilities:
- Prepare, verify, and dispense complex intravenous (IV) medications including hazardous drugs, TPN, IVIg, and biologics, strictly following USP 797/800 guidelines.
- Conduct clinical evaluations of medication orders for appropriateness, dosage, interactions, and compatibility, with a focus on high-acuity conditions such as oncology, immunology, and transplant.
- Monitor patient responses to therapies, adjusting care plans as necessary to improve outcomes and manage side effects.
- Educate patients about medication administration, therapy expectations, and adverse effect management to promote adherence and safety.
- Ensure regulatory compliance through meticulous documentation and maintaining cleanroom environments.
- Collaborate with healthcare providers including nurses, physicians, and case managers to coordinate patient care and manage prior authorizations effectively.
Desired Qualifications:
- Current, valid Ohio pharmacist license.
- Experience in sterile compounding and preparation of IV medications.
- Strong clinical knowledge in managing complex therapies and chronic diseases.
- Familiarity with USP 797/800 standards and regulatory requirements.
- Excellent communication and teamwork skills.
