A clinical pharmacist specializing in home infusion plays a critical role in preparing, dispensing, and monitoring complex intravenous (IV) and subcutaneous medications for patients receiving care at home. This position acts as the central coordinator within a multidisciplinary team, ensuring safe sterile compounding, precise medication adjustments, and thorough patient education.

Core Responsibilities:

  • Prepare specialty pharmaceuticals and IV solutions under sterile, ventilated conditions, complying with all state and federal regulations.
  • Assess laboratory results, perform pharmacokinetic dosing calculations (e.g., vancomycin), and monitor Total Parenteral Nutrition (TPN) to optimize treatments.
  • Counsel patients and caregivers on specialty medications, infusion pump operation, sterile techniques, and catheter maintenance.
  • Collaborate with prescribing physicians, home health nurses, and internal teams—including intake, referral, reimbursement, nursing, clinical services, and pharmacy operations—to enhance therapy management and reduce hospital readmissions.
  • Manage compliance with regulatory standards, including those set by State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, and OSHA.
  • Oversee inventory management, including refrigerated products, and ensure safe, timely packaging and shipping of medications to patients’ homes.

Qualifications / Desired Experience:

  • Minimum 2 years of infusion specialty pharmacy experience.
  • Experience with IVIg therapy is a plus.
  • Strong understanding of specialty therapy payer requirements, reimbursement processes, and contract obligations.
  • Current and in good standing NY pharmacist license.
  • Experience using CareTend pharmacy system preferred.
