A clinical pharmacist specializing in home infusion therapy plays a pivotal role in preparing, dispensing, and monitoring complex intravenous (IV) and subcutaneous medications for patients. This position serves as the central hub within a multidisciplinary team, ensuring safe sterile compounding, appropriate medication dose adjustments, and providing essential education to patients and caregivers.

Responsibilities include:

• Sterile Compounding: Prepare specialty pharmaceuticals and IV solutions in a sterile, ventilated environment, adhering to all state and federal regulations.
• Clinical Monitoring: Assess laboratory values, calculate pharmacokinetic dosing (for example, vancomycin), and monitor Total Parenteral Nutrition (TPN) treatments. Provide detailed patient counseling on specialty medications and monitor therapies for potential issues, recommending clinical interventions when necessary.
• Patient Onboarding & Education: Instruct patients and caregivers on infusion pump use, sterile techniques, and catheter maintenance at home.
• Care Coordination: Work closely with prescribing physicians, home health nurses, and internal teams including intake, referral services, reimbursement, nursing, clinical services, and pharmacy operations to ensure smooth therapy management and reduce hospital readmissions.
• Regulatory Compliance: Maintain compliance with State Board of Pharmacy regulations and regulatory standards including HIPAA, FDA, DEA, Medicare, ACHC, URAC, and OSHA.
• Inventory & Shipping: Manage pharmacy inventory, particularly refrigerated products, and prepare medications and supplies for safe, timely shipment to patients' homes.

Qualifications:

• Minimum of 2 years’ experience in infusion specialty pharmacy.
• IVIg experience is highly desirable.
• Strong knowledge of specialty therapy payer requirements, reimbursement processes, and contract obligations.
• Current, valid NY pharmacist license in good standing.
• Familiarity with CareTend system preferred.
