Clinical Pharmacist - Dublin, OH

A clinical pharmacist specializing in home infusion plays a vital role in managing complex intravenous (IV) and subcutaneous therapies for patients in the comfort of their homes. This position demands expertise in sterile compounding, clinical monitoring, patient education, and seamless coordination with healthcare teams to ensure optimal therapeutic outcomes.

Key Responsibilities:

• Sterile Compounding: Prepare specialty pharmaceuticals and IV solutions in a sterile, ventilated environment adhering strictly to state and federal guidelines.
• Clinical Monitoring: Evaluate lab results, perform pharmacokinetic dosing calculations (e.g., vancomycin), and monitor Total Parenteral Nutrition (TPN), adjusting treatment plans accordingly. Provide detailed patient counseling regarding specialty medications and therapy.
• Patient Onboarding & Education: Train patients and caregivers on infusion pump operation, sterile technique, and catheter care to ensure safe at-home infusion treatments.
• Care Coordination: Work collaboratively with prescribing physicians, home health nurses, intake, referral services, reimbursement specialists, nursing, clinical services, and pharmacy operations to deliver comprehensive care and minimize hospital readmissions.
• Regulatory Compliance: Maintain strict adherence to all applicable regulations, including those by State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, and OSHA.
• Inventory & Shipping: Manage pharmacy inventory, including temperature-sensitive products, and oversee accurate packaging and timely shipping of medications to patients’ homes.

Qualifications:

• Minimum of 2 years’ experience in infusion specialty pharmacy.
• IVIg experience is a plus.
• Strong knowledge of specialty therapy payer requirements, reimbursement processes, and contract obligations.
• Current and valid Ohio pharmacist license in good standing.
• Proficiency with CareTend system preferred.
