Home Infusion Clinical Pharmacist | Fishers, Indiana

A home infusion clinical pharmacist opportunity is available in the greater Indianapolis, IN area with a direct hire placement. This role, based in Fishers, IN, offers the chance to apply specialized pharmacy skills in a dynamic home infusion setting. The position supports patients by preparing, dispensing, and monitoring complex intravenous (IV) and subcutaneous medications, while providing education and care coordination to improve therapy outcomes.

Schedule:

• Monday to Friday, 8:30 a.m. to 5:00 p.m.

Key Responsibilities:

• Prepare specialty pharmaceuticals and IV solutions in a sterile environment, adhering to all state and federal regulations.
• Assess lab values and calculate pharmacokinetic dosing (e.g., vancomycin) while monitoring Total Parenteral Nutrition (TPN), adjusting treatments as needed.
• Provide patient counseling and education on specialty medications, infusion pump use, sterile techniques, and catheter maintenance.
• Collaborate with prescribing physicians, home health nurses, and internal teams to ensure smooth patient care and reduce hospital readmissions.
• Maintain compliance with regulations including State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, and OSHA.
• Manage pharmacy inventory, including refrigerated products, and prepare medications and supplies for safe, prompt home delivery.

Preferred Experience and Qualifications:

• Minimum of 2 years infusion specialty pharmacy experience.
• IVIg experience is a plus.
• Understanding of specialty therapy payer requirements, reimbursement processes, and contract obligations.
• Familiarity with CareTend system.
• Valid Indiana pharmacist license in good standing.
