Per Diem Clinical Pharmacist - Fontana, California

This opportunity calls for a Clinical Pharmacist with solid inpatient experience to support a hospital setting serving a med-surg and emergency department unit. The ideal candidate will provide expert medication management, compound sterile preparations, and ensure compliance with regulatory standards while working a defined weekday schedule and on-call hours.

Key Responsibilities:

• Verify and review medication orders for accuracy, appropriateness, and patient safety
• Compound sterile medications such as IV admixtures and unit dose preparations (non-chemotherapy/hazardous)
• Respond promptly to emergency room codes as needed
• Monitor patient medication therapy and collaborate with physicians and nurses to optimize treatment
• Maintain adherence to hospital policies, state, and federal regulations, including sterile compounding standards
• Assist with filling Pyxis automated dispensing machines at associated family health centers

Qualifications:

• Active and clear California Pharmacist License (required)
• Minimum 2+ years of inpatient pharmacy experience
• Experience with clinical pharmacy software such as CPSI/Evident and Pyxis preferred; training available if needed
• Prior experience functioning independently as the sole pharmacist in a clinical setting is a plus
• Ability and willingness to work on-call hours, including weekends and after hours

Location and Schedule:

• Based in Fontana, CA
• Per diem status with full-time coverage Monday through Friday, 9:00 AM to 5:00 PM
• On-call availability for weekends and after hours, with infrequent calls expected

Additional Support:

• Access to an experienced PRN pharmacy technician during the contract to assist with inventory and data entry
