Clinical Pharmacist | Glenwood Springs, Colorado

An experienced clinical pharmacist is needed to support patient care in a hospital setting located in Glenwood Springs, CO. This contract role offers a rotating schedule with 40 hours per week, including early morning and weekend shifts. The assignment starts as soon as possible with a projected end date of 8/15/26.

Location:

• Glenwood Springs, CO

Schedule:

• Rotating day/evening shifts, Monday through Friday starting at either 6:30 AM or 8:00 AM
• Rotating weekends working 7:00 AM to 12:00 PM

Key Responsibilities:

• Prepare and dispense medication orders as requested by physicians
• Maintain medication stock within patient care areas
• Ensure safe, effective, and cost-conscious medication therapies adhering to established protocols
• Interpret medication orders and maintain computerized medication profiles
• Compound and dispense pharmaceutical products, including IV admixtures and chemotherapy
• Monitor drug therapy for contraindications, interactions, and dose appropriateness
• Perform pharmacokinetic dosing and antibiotic stewardship
• Provide clinical consultations and alternative therapeutic recommendations
• Document all clinical activities accurately
• Participate in quality improvement and educational programs
• Serve as a member of the Code Response Team

Qualifications:

• Active Colorado Pharmacist License in good standing
• Graduate from an ACPE-accredited School of Pharmacy with a Bachelor of Science in Pharmacy
• Current BLS and ACLS certifications from the American Heart Association
• Required flu vaccination during flu season (October 1–April 1), no exemptions allowed
• Must have Epic electronic medical records experience; Meditech training is a plus for the facility’s EMR conversion period
