A Long-Term Acute Care (LTAC) pharmacy clinician opportunity is available in Marin County, CA, supporting a patient-centered hospital environment. This contract role offers part-time hours with flexibility and potential to transition to full-time. The position is ideal for a pharmacist wanting to enhance clinical expertise within a smaller hospital setting focused on complex patient care.
Schedule/Hours:

Responsibilities:
- Review and verify medication orders in an LTAC hospital
- Support clinical pharmacy services including pharmacokinetics and anticoagulation monitoring when applicable
- Assist with medication dispensing and automation workflows via Pyxis
- Collaborate with interdisciplinary care teams to ensure optimal patient outcomes
- Manage safe and effective medication use for medically complex patients
Qualifications:
- Bachelor of Science or Doctor of Pharmacy (PharmD) from an accredited School of Pharmacy
- Active and unrestricted California Pharmacist License in good standing
Preferred Experience:
- 2–3 years of retail pharmacy experience with a strong interest in clinical/hospital transition
- Prior inpatient or hospital pharmacy familiarity
- Experience with TPN preparation and monitoring
- Knowledge of pharmacokinetics and anticoagulation management
- Current BLS and ACLS certifications are preferred
