Per Diem Clinical Pharmacist | Highland, California

Support inpatient pharmacy operations in a dynamic hospital setting, ensuring the safe and effective use of medications. This per diem role based in Highland, CA offers a weekday day shift schedule with minimal on-call responsibilities during weekends and after hours.

Schedule:

• Monday to Friday, 9:00 AM – 5:00 PM (Full-time hours)
• On-call weekends and after hours (rarely called in)

Responsibilities:

• Verify and review medication orders for accuracy, appropriateness, and patient safety
• Compound sterile medications, including IV admixtures and unit dose medication (no chemotherapy or hazardous drugs)
• Respond to emergency room codes as needed, supporting critical patient care situations
• Ensure adherence to hospital policies, state and federal regulations, and sterile compounding standards
• Monitor patients’ medication therapy, providing clinical recommendations to physicians and nursing staff
• Replenish Pyxis machines at two family health centers within the local community

Qualifications:

• Active and clear California Pharmacist License
• Minimum of 2 years inpatient pharmacy experience
• Familiarity with clinical software systems such as CSPI, Evident, or Pyxis preferred (training available if needed)
• Prior experience as the sole pharmacist in a clinical environment is advantageous
• Willingness and ability to fulfill all on-call hour requirements
