Clinical Pharmacist - Houston, Texas

An opportunity is available for a Clinical Pharmacist to provide expert pharmaceutical care at a women's health facility in Houston, TX. This contract position offers an overnight shift on a 7-on/7-off schedule from 7:30 PM to 7:30 AM, starting May 18 for approximately 14 weeks.

Responsibilities include managing the medication process to promote safe pharmaceutical care, preventing and resolving drug-related issues, and meeting the professional standards as required by Texas Children's Hospital. The role requires working with patients ranging from newborns to adults, ensuring medication appropriateness based on diagnosis, weight, allergies, and drug interactions, and accurately processing medication orders using the EPIC electronic medical record system.

Key duties:

• Verify and confirm medication doses are appropriate and safe.
• Accurately enter and process medication orders to avoid errors.
• Ensure timely delivery of medications in compliance with guidelines.
• Maintain aseptic technique and infection control during sterile product preparation; complete annual certification with a minimum 90% score.
• Collaborate with technicians and pharmacists to uphold high standards of practice.

Qualifications and Desired Experience:

• Minimum 2 years of inpatient pharmacy experience; pediatric experience highly preferred.
• Valid Texas pharmacist license.
• ACPE IV Certification required.
• Proficiency with EPIC EMR system is a plus.
• Strong attention to detail and commitment to patient safety.
