Schedule:

• One week: 7:00 AM – 3:30 PM
• Alternate week: 12:00 PM – 8:00 PM

This opportunity is for a Clinical Pharmacist specializing in sterile compounding, focused on home infusion therapy services. The role requires a thorough understanding of sterile medication preparation, patient evaluation, and clinical monitoring to ensure safe and effective treatment.

Responsibilities:

• Evaluate patients for home infusion therapy eligibility and develop individualized medication plans.
• Prepare, verify, and dispense sterile medications following USP standards and regulatory requirements.
• Educate patients, caregivers, and healthcare providers on medication use and infusion administration.
• Collaborate with healthcare providers to monitor patient outcomes and ensure continuity of care.
• Manage infusion equipment and maintain quality assurance and safety protocols.
• Provide accurate drug information, support regulatory compliance, and maintain documentation.

Qualifications / Desired Experience:

• Doctor of Pharmacy (PharmD) or graduate of an accredited School of Pharmacy.
• Active, unrestricted Texas Pharmacist license required.
• At least one of the following in sterile compounding or related experience:
  • Minimum 1 year sterile IV compounding, or
  • Experience with non-sterile compounding, immunizations, and MTM, or
  • Completion of an ASHP-accredited Pharmacy Residency, or
  • Completion of an Advanced Pharmacy Practice Experience (APPE), or
  • Equivalent pharmacy experience.
• Strong clinical knowledge of home infusion pharmacy practices including pharmacokinetics and laboratory value interpretation.
• Excellent communication and patient counseling skills.
