This direct hire opportunity seeks a Clinical Pharmacist specialized in home infusion therapies to provide expert pharmaceutical care in a dynamic setting. The role focuses on managing complex intravenous (IV) and subcutaneous medications, ensuring patient safety, and facilitating optimal therapeutic outcomes.

Responsibilities:

  • Prepare specialty pharmaceuticals and IV solutions in a sterile environment, adhering to state and federal regulations.
  • Monitor clinical parameters such as lab values and pharmacokinetic dosing (e.g., vancomycin), and adjust Total Parenteral Nutrition (TPN) treatments accordingly.
  • Deliver comprehensive education to patients and caregivers on the use of infusion pumps, sterile techniques, and catheter maintenance.
  • Collaborate with physicians, home health nurses, and multidisciplinary teams including intake, referral services, reimbursement, nursing, clinical services, and pharmacy operations to ensure coordinated patient care and reduce hospital readmissions.
  • Maintain strict compliance with all relevant regulatory standards including State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, and OSHA.
  • Oversee pharmacy inventory management, including refrigerated products, and prepare medications and supplies for secure and prompt home delivery.

Qualifications and Desired Experience:

  • Minimum of 2 years experience in infusion specialty pharmacy.
  • Familiarity with IVIg therapies is highly desirable.
  • Strong knowledge of specialty therapy payer requirements, reimbursement procedures, and contract obligations.
  • Current and in good standing NY pharmacist license.
  • Experience with CareTend system preferred.
