Per Diem Clinical Pharmacist in Manteca, CA

A per diem clinical pharmacist position is available to support infusion pharmacy services at an outpatient site in Northern California, working 2-3 days per week, Monday through Friday from 8:30 AM to 5:00 PM. This role is ideal for candidates based near Modesto, Stockton, or Merced and requires an immediate start with ongoing assignment duration.

Responsibilities include:

• Reviewing and evaluating physician/prescriber orders for dosage appropriateness, drug choice related to diagnosis, potential interactions, administration route, and stability.
• Ensuring patient records are up to date with height, weight, medical history, labs, and medication details, including OTC and supplements.
• Maintaining all documentation to comply with state, federal, and accreditation requirements.
• Ordering, reviewing, and discussing clinical laboratory tests with nurses and physicians to assure safe and effective therapy.
• Providing clinical and technical support to physicians, nurses, and patients.
• Assisting with policy implementation and training/supervision of pharmacy staff including technicians and interns.
• Supervising all sterile compounding activities and quality assurance protocols.
• Documenting all communications within patient medical records.
• Educating patients and caregivers on drug therapies and safety.

Qualifications and Desired Experience:

• Active California pharmacist license in good standing.
• Strong clinical, IV, and infusion pharmacy expertise.
• Prior home infusion pharmacy experience highly preferred.
• Familiarity with CareTend pharmacy management system is a plus.
