A healthcare facility in Manteca, CA is seeking a Per Diem Clinical Pharmacist with specialized experience in infusion and home infusion pharmacy to provide part-time support. This opportunity is ideal for candidates located in or near Modesto, Stockton, or Merced. The position offers a consistent schedule of 2-3 days per week, Monday through Friday from 8:30 a.m. to 5:00 p.m., with an immediate start and ongoing duration.

Key Responsibilities:

  • Review and evaluate physician/prescriber orders for dosage accuracy, drug selection, route of administration, and stability within the context of the patient’s diagnosis.
  • Maintain and verify the completeness of patient records including current height, weight, detailed medical history, lab results, and comprehensive medication profiles.
  • Ensure compliance with documentation requirements set by state and federal agencies, as well as accrediting bodies like JCAHO and ACHC.
  • Order, assess, and discuss clinical laboratory test results with nursing staff and physicians to ensure safe and effective medication therapy.
  • Provide clinical and technical support to physicians, nurses, and patients.
  • Assist the pharmacist-in-charge with implementation of policies and procedures.
  • Participate in training and supervision of pharmacy technicians, interns, and laboratory personnel.
  • Oversee sterile compounding activities and lead quality assurance programs including validation and environmental testing.
  • Accurately document all communications with healthcare professionals and patients in medical records.
  • Promote patient safety through adherence to best pharmacy practices.
  • Educate patients and caregivers on drug therapies and instructions.

Qualifications and Experience:

  • Active California pharmacist license in good standing.
  • Strong clinical, IV, and infusion pharmacy expertise.
  • Prior home infusion pharmacy experience preferred.
  • Proficiency with CareTend system.
