Clinical Pharmacist - Merced, California

An infusion pharmacist role is available in Merced, CA, offering part-time hours (2-3 days per week) with a Monday through Friday schedule, 8:30 a.m. to 5:00 p.m. Start immediately and engage in ongoing work supporting infusion services. This position welcomes candidates with infusion and clinical pharmacy experience, particularly those skilled in IV and TPN management. New graduates within the last 2-3 years with relevant APPE or internship experience are encouraged to apply, as training is provided to build proficiency in hospital IV/TPN practices.

Key Responsibilities:

• Review physician prescription orders for appropriateness including dosage, drug choice, interactions, administration routes, and stability.
• Maintain complete and up-to-date patient databases including medical history, lab results, and current medications.
• Support compliance with paperwork and documentation required by regulatory and accrediting bodies.
• Coordinate clinical lab testing and collaborate with healthcare professionals regarding results.
• Provide clinical and technical support to physicians, nurses, and patients.
• Assist in implementing pharmacy policies, supervising pharmacy personnel, and overseeing sterile compounding and quality assurance programs.
• Document communications with healthcare team members and patients thoroughly.
• Promote patient and staff safety through best pharmacy practices.
• Deliver drug therapy instructions and education to patients and caregivers.

Qualifications:

• Current California pharmacist license in good standing.
• Strong infusion, IV, and clinical pharmacy experience; home infusion pharmacy experience is highly preferred.
• Excellent communication skills and demonstrated teamwork mindset.
• Organized and professional approach to resume details, including clear timelines and responsibilities.
• Familiarity with CareTend system is a plus.
