Clinical Pharmacist - Middleton, Wisconsin

A healthcare facility in Middleton, WI is seeking three inpatient pharmacists for a 13-week contract starting June 29. Each position covers one of the following shifts, working five 8-hour shifts per week with rotating every third weekend:

• Day shift: 6:30 a.m. to 3:00 p.m.
• Mid shift: 11:00 a.m. to 7:30 p.m.
• Evening shift: 2:30 p.m. to 11:00 p.m.

Responsibilities:

• Provide clinical pharmacy services within an inpatient/acute care setting
• Perform pharmacokinetic and renal dosing
• Conduct therapeutic medication monitoring
• Manage drug shortages
• Deliver pharmacotherapy management for various medical conditions
• Utilize EPIC electronic medical records system for documentation and communication

Qualifications:

• Minimum of 2 years recent inpatient pharmacy experience required
• Current pharmacy license in Wisconsin (ACT-10 licensure acceptable)
• Proficiency with EPIC EMR system

• Travelers must comply with mask fit testing requirements, which include being clean-shaven; the use of personal CAPRs or PAPRs is prohibited, and the facility will provide scrubs upon starting.
