Clinical Pharmacist in Middleton, WI

A healthcare facility in the Middleton, WI area is seeking three experienced inpatient pharmacists for a 13-week contract starting June 29. These clinical pharmacists will support acute care operations across day, mid, and evening shifts, covering essential medication management in a hospital setting. Schedule:

• One Day Shift: 6:30 AM – 3:00 PM
• One Mid Shift: 11:00 AM – 7:30 PM
• One Evening Shift: 2:30 PM – 11:00 PM
  Each shift follows a 5-day, 8-hour schedule with rotation every third weekend.

Key Responsibilities:

• Provide comprehensive pharmacotherapy management for medical inpatients
• Conduct pharmacokinetic and renal dosing adjustments
• Perform therapeutic medication monitoring and manage drug shortages
• Utilize the EPIC EMR system for clinical documentation and order verification
• Collaborate with healthcare teams to ensure optimal patient outcomes

Required Qualifications:

• Minimum of 2 years recent inpatient or acute care pharmacy experience
• Proficiency with EPIC electronic medical record system (mandatory)
• Valid Wisconsin pharmacy license; ACT-10 license holders are eligible

Important Notes:

• Travelers must be able to pass Fit Testing for N95 mask seal; personal PAPRs and CAPRs are not permitted
• Failure to comply with Fit Testing requirements will result in assignment cancellation
• Scrubs will be provided upon start date
