This clinical pharmacy role offers an opportunity to deliver advanced ambulatory care pharmacy services as a credentialed and privileged member of a dynamic healthcare team. The specialist will operate under Collaborative Practice Agreements (CPAs) to optimize therapeutic outcomes, improve medication access, and reduce overall care costs. This high-impact position serves as a clinical resource within the ambulatory network, supporting Comprehensive Medication Management (CMM), chronic disease management, and specialty medication care coordination.

Responsibilities:

  • Provide comprehensive ambulatory clinical pharmacy services including medication assessment, monitoring, and documentation under pharmacy CPAs.
  • Conduct patient visits involving medication reconciliation, laboratory monitoring, patient counseling, and disease state management.
  • Collaborate with multidisciplinary care teams to implement evidence-based medication interventions aimed at quality, safety, and cost-efficiency.
  • Manage referrals with clear communication back to providers to ensure continuity of care.
  • Facilitate patient access to medications through prior authorizations, benefit investigations, and financial assistance support.
  • Oversee specialty medication protocols including initiation, monitoring, and ongoing therapy management.
  • Serve as a resource for drug information and education to patients, families, and healthcare professionals.
  • Precept pharmacy students, interns, and residents as assigned.
  • Participate in program development, research, quality improvement initiatives, and committee work to advance ambulatory pharmacy services.
  • Document interventions and outcomes within the electronic health record consistent with established standards.

Qualifications and Desired Experience:

  • Active Delaware Pharmacist License in good standing (or willingness to obtain).
  • Board of Pharmacy Specialties (BPS) Certification is required in Pharmacotherapy, Ambulatory Care, Cardiology, or Geriatrics.
  • Minimum of one year clinical pharmacy experience, including residency.
  • Bachelor of Science in Pharmacy or Doctor of Pharmacy (PharmD strongly preferred).
  • Collaborative Practice Agreement experience preferred.
  • Experience managing specialty medications and conducting prior authorizations desirable.
  • Ambulatory care or population health pharmacy background a strong plus.
  • PGY1 or PGY2 ambulatory care residency completion highly advantageous.
  • Precepting experience welcomed.
