Clinical Pharmacist - Milwaukee, WI

A healthcare facility in Wisconsin is seeking experienced inpatient pharmacists to provide clinical pharmacy services in a hospital setting. This contract position offers an opportunity to apply specialized skills in acute care and pharmacotherapy within a respected inpatient environment.

Location: Milwaukee, WI (assignment at a hospital in nearby Madison, WI)
Job Type: Contract (13 weeks)
Start Date: June 8
Schedule:

• Mix of day shifts (6:30 a.m. – 3:00 p.m.) and evening shifts (2:30 p.m. – 11:00 p.m.)
• 5 shifts per week, 8 hours each
• Rotating every 3rd weekend

Responsibilities:

• Provide inpatient clinical pharmacy services including therapeutic medication monitoring and pharmacotherapy management
• Utilize expertise in pharmacokinetic and renal dosing to optimize patient outcomes
• Manage drug shortages effectively in collaboration with the healthcare team
• Use EPIC electronic medical record system to document and review medication profiles
• Collaborate with physicians and nursing staff to ensure safe and effective medication use

Qualifications:

• Minimum 2 years recent inpatient pharmacy experience
• Proficient in acute care pharmacotherapy principles, dose adjustments, and emergency medication protocols
• Licensed pharmacist in Wisconsin or eligible for ACT-10 license
• Ability to pass fit testing for mask seal; must be clean-shaven as the facility does not allow personal PAPRs or CAPRs
• Comfortable with rotating shift schedules and weekend work
