Per Diem Clinical Pharmacist in Modesto, CA

A healthcare facility in Modesto, CA, is seeking a per diem Clinical Pharmacist to provide infusion pharmacy support 2-3 days per week, Monday through Friday from 8:30 a.m. to 5 p.m. This opportunity offers ongoing work with a quick start, ideal for candidates located in Modesto, Stockton, or Merced.

Responsibilities include:

• Reviewing physician/prescriber orders for dosage accuracy, drug appropriateness related to the patient’s diagnosis, potential interactions, and administration routes.
• Ensuring patient records are complete with up-to-date height, weight, medical history, lab results, and current medications, including OTC and supplements.
• Maintaining compliance documentation in line with state, federal, and accrediting agencies such as JCAHO and ACHC.
• Coordinating clinical lab tests, reviewing results for medication safety and effectiveness, and collaborating with nurses and physicians.
• Providing clinical and technical support to healthcare professionals and patients.
• Assisting in policy implementation and supervising pharmacy staff including technicians and interns.
• Overseeing sterile compounding and quality assurance programs like validation and environmental testing.
• Documenting all healthcare communications clearly within patient medical records.
• Promoting safe pharmacy practices and participating in patient drug therapy education.

Qualifications:

• CA pharmacist license in good standing.
• Extensive IV/infusion and clinical pharmacy experience.
• Previous home infusion pharmacy experience strongly preferred.
• Familiarity with CareTend system.
