Per Diem Clinical Pharmacist | Modesto, California

A per diem clinical pharmacist opportunity is available to support a busy home infusion site near Modesto, CA. This role offers consistent part-time hours, working 2 to 3 days a week, Monday through Friday from 8:30 a.m. to 5:00 p.m., with immediate start and ongoing engagement. Candidates from Modesto, Stockton, or Merced are ideal.

Key Responsibilities:

• Review and evaluate physician prescription orders for appropriate dosage, drug choice, administration route, and potential interactions.
• Maintain thorough patient records, including current height, weight, medical history, lab results, and concurrent medications.
• Support regulatory compliance by assisting with documentation for state and federal requirements and accrediting agencies.
• Coordinate with healthcare team members by ordering and assessing clinical laboratory tests relevant to patient safety and medication effectiveness.
• Provide clinical and technical support to physicians, nurses, and patients.
• Assist pharmacist-in-charge in updating policies, procedures, and overseeing sterile compounding and quality assurance programs.
• Train and supervise pharmacy technicians, interns, and other personnel.
• Document all professional communications in patient medical records.
• Promote patient and staff safety through adherence to best pharmacy practices.
• Participate actively in educating patients and caregivers about drug therapies.

Qualifications / Desired Experience:

• Valid California pharmacy license in good standing.
• Strong IV/infusion and clinical pharmacy experience required.
• Previous experience in home infusion pharmacy highly preferred.
• Familiarity with CareTend pharmacy system is beneficial.

Location:

• Based near Modesto, CA, with candidates from nearby cities preferred.
