Per Diem Pharmacist Clinical | Modesto, California

A healthcare facility is seeking a skilled Clinical Pharmacist to provide part-time infusion pharmacy support at their site. This role offers an opportunity to contribute 2-3 days per week during regular business hours, Monday through Friday from 8:30 a.m. to 5 p.m., with a start date as soon as possible and ongoing assignment duration. Candidates based in Modesto, Stockton, or Merced are encouraged to apply.

Key Responsibilities:

• Review and evaluate physician or prescriber orders to ensure appropriate dosage, drug selection, and administration in relation to the patient’s diagnosis and condition.
• Maintain and verify comprehensive patient records, including medical history, lab results, medication lists, and other relevant health data.
• Support compliance efforts by assisting with required documentation for state, federal, and accrediting bodies such as JCAHO and ACHC.
• Order and interpret clinical laboratory tests, collaborating with nurses and physicians to assess treatment safety and efficacy.
• Provide clinical and technical assistance to healthcare providers and patients.
• Implement new pharmacy policies and procedures alongside the pharmacist-in-charge.
• Train and oversee pharmacy technicians, interns, laboratory staff, and others involved in pharmacy operations.
• Supervise sterile compounding processes and quality assurance activities including validation and environmental testing.
• Document all professional communications with healthcare team members and patient caregivers accurately in patient records.
• Promote safe pharmacy practices to protect patients and staff.
• Deliver drug therapy education and instructions to patients and caregivers as part of comprehensive care.

Qualifications and Experience:

• Current California pharmacist license in good standing.
• Demonstrated proficiency in IV, infusion, and clinical pharmacy, with strong preference for candidates with home infusion pharmacy experience.
• Familiarity with CareTend pharmacy management system is a plus.
• Excellent attention to detail and effective communication skills.
