Clinical Pharmacist | Modesto, California

An infusion pharmacist opportunity is available offering part-time support (2-3 days/week) with a schedule of Monday through Friday, 8:30 a.m. to 5:00 p.m. This position is based in Modesto, CA, with an ASAP start and ongoing duration. Recent graduates with IV/TPN experience during APPEs or internships are encouraged to apply, as training is provided for those willing to learn hospital IV/TPN protocols.

Key Responsibilities:

• Review physician/prescriber orders for dosage accuracy, drug choice appropriateness, potential interactions, administration route, and stability.
• Maintain a complete patient database including medical history, lab results, and current medications, covering prescription, over-the-counter, herbal, and nutritional supplements.
• Support compliance by maintaining documentation for state, federal, and accrediting agencies.
• Collaborate with healthcare staff to order and assess clinical lab tests related to medication safety and effectiveness.
• Provide clinical and technical support to physicians, nurses, and patients.
• Assist in developing and implementing pharmacy policies and procedures.
• Participate in training and supervising pharmacy technicians, interns, and lab staff.
• Oversee sterile compounding processes and quality assurance programs.
• Document all professional communications in patient medical records.
• Ensure patient and personnel safety through good pharmacy practices.
• Educate patients and caregivers on drug therapy instructions.

Desired Qualifications:

• Strong experience with IV/infusion and clinical pharmacy practices.
• Prior home infusion pharmacy experience highly preferred.
• Valid California pharmacist license in good standing.
• Excellent communication skills with a team-oriented attitude.
• Commitment to professional resume presentation, including detailed, date-specific duties.

Systems Experience:

• Familiarity with CareTend system is beneficial.
