Responsibilities:

  • Provide clinical pharmacy services including medication therapy management and patient counseling
  • Review and verify medication orders for accuracy and appropriateness within a hospital setting
  • Collaborate with physicians, nurses, and other healthcare professionals to optimize drug therapy outcomes
  • Monitor patient progress and response to medications, adjusting therapies as needed
  • Ensure compliance with hospital policies, safety standards, and regulatory requirements
  • Participate in clinical rounds and contribute to interdisciplinary care planning

Qualifications / Desired Experience:

  • Licensed Pharmacist with experience in clinical pharmacy and hospital pharmacy operations
  • Strong knowledge of medication therapies, drug interactions, and hospital protocols
  • Ability to work effectively in a multidisciplinary healthcare team
  • Excellent communication and organizational skills
  • Experience with electronic medical records and pharmacy information systems preferred
