Per Diem Clinical Pharmacist - Pittsburgh, Pennsylvania

A per diem opportunity is available for a Clinical Pharmacist specializing in long-term care. This role involves working 24 hours per week within a structured Monday through Friday schedule, supporting skilled nursing facilities, personal care facilities, and hospice services.

Key Responsibilities:

• Review and verify medication orders entered by data entry personnel for long-term care patients.
• Inspect packaged medications, including blister packs, as well as sterile and non-sterile preparations.
• Respond to clinical inquiries from nursing facilities, hospice nurses, and physicians via phone.
• Perform controlled substance counts and maintain accurate perpetual inventory documentation.
• Conduct checks on emergency medication boxes to ensure readiness and compliance.

Qualifications & Experience:

• Bachelor’s degree in Pharmacy.
• Active pharmacist license in good standing with the Pennsylvania State Board.
• Minimum of 1 year pharmacy experience; long-term care experience preferred.
• Proficient with pharmacy operating systems such as QS1, Docutrack, and PCC.
• Ability to perform sitting, standing, and fine motor tasks; visual acuity required.
• Capability to lift up to 30 pounds as needed.

Schedule:

• 24 hours per week.
• Pharmacy operational hours: Monday through Friday, 8 am – 6 pm.
