A healthcare facility in State College, PA is seeking a Clinical Pharmacist to join their team on a permanent basis, with potential for temp-to-hire arrangements. Key Responsibilities:

• Collaborate with the healthcare team to optimize medication therapy management, especially in targeted disease states
• Provide direct patient care in ambulatory settings and support population health initiatives
• Assist with pharmacy staffing and operational support
• Utilize Meditech for documentation and workflow management
• Contribute to the growth and development of clinical pharmacy services within the hospital

Desired Qualifications:

• Active Pennsylvania pharmacist license
• Experience in ambulatory care or inpatient pharmacy settings
• Completion of a residency program is highly preferred
• Strong communication skills and commitment to patient-centered care

Schedule: Variable days and afternoon shifts, primarily 8-hour day shifts
Information Technology: Meditech system experience preferred
