Clinical Pharmacist

We're seeking an experienced Clinical Pharmacist for a hospital assignment in Portland, Oregon. If you're looking for a fast-paced inpatient environment where you can utilize your clinical expertise and collaborate with an interdisciplinary care team, this is an outstanding opportunity.

What You'll Do

• Verify and review medication orders for accuracy and appropriateness
• Provide clinical pharmacy services to hospitalized patients
• Collaborate with physicians, nurses, and other healthcare professionals to optimize medication therapy
• Perform pharmacokinetic dosing and therapeutic drug monitoring
• Participate in clinical interventions and medication safety initiatives
• Utilize Cerner and Pyxis for daily pharmacy operations
• Provide drug information and clinical recommendations to the care team

Requirements

• Active Oregon Pharmacist license
• Strong recent inpatient clinical hospital pharmacy experience
• Experience with order verification and clinical pharmacy services
• Experience with Cerner and Pyxis preferred
• Excellent communication and critical thinking skills
