An inpatient clinical pharmacist is needed for a 13-week contract assignment starting Tuesday, May 26, at a medical center in Portsmouth, VA. This role supports rotating shifts and requires expertise in inpatient pharmacy services with a focus on clinical patient care.
Key Responsibilities:
- Verify and enter medication orders accurately using EPIC electronic medical records (EMR)
- Manage IV medications, pain-controlled analgesia, and antibiotic therapies
- Monitor drug interactions and adjust therapy as necessary
- Provide specialized dosing for anticoagulants, pharmacokinetics, and renal impairments (preferred)
- Participate in chemotherapy order verification (preferred)
- Collaborate with healthcare team members to optimize patient outcomes
Qualifications & Desired Experience:
- Minimum one year of inpatient pharmacy experience is required
- Proficiency with EPIC EMR system is mandatory
- Experience with intravenous medication management, pain-controlled analgesia, antibiotics, and drug interaction assessment
- Travel pharmacy experience is preferred but not required
- Familiarity with chemotherapy, anticoagulants, pharmacokinetic dosing, and renal dosing is highly valued
