A part-time Pharmacist position is available to support a Long-Term Acute Care (LTAC) hospital in Marin County, California. This role offers the opportunity to work in a clinical inpatient setting within a smaller, patient-centered environment. Responsibilities:
- Review and verify medication orders specific to LTAC hospital settings
- Provide clinical pharmacy services, including pharmacokinetics and anticoagulation monitoring as applicable
- Assist with medication dispensing and manage automation workflows using Pyxis systems
- Collaborate with interdisciplinary teams to optimize medication therapy and improve patient outcomes
- Ensure safe, effective medication management for medically complex patients
Qualifications:
- Bachelor of Science or Doctor of Pharmacy (PharmD) from an accredited institution
- Active, unrestricted California Pharmacist License in good standing
Preferred Experience:
- 2–3 years of retail pharmacy experience with a demonstrated interest in clinical or inpatient pharmacy roles
- Prior experience in inpatient or hospital pharmacy practice
- Knowledge/experience in TPN preparation and monitoring
- Understanding of pharmacokinetics and anticoagulation management
- Current Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications are a plus
