Sterile Compounding Pharmacist

13-Week Contract | Schedule: 12:00 PM - 8:00 PM

We are seeking a Sterile Compounding Pharmacist to support a home infusion pharmacy team. This role combines clinical pharmacy expertise, sterile compounding, and patient-centered care to ensure safe and effective medication therapy for patients receiving treatment at home.

What You'll Do

  • Evaluate patients for home infusion therapy and determine eligibility for home-based treatment.
  • Develop, monitor, and optimize individualized medication therapy plans using clinical assessments, laboratory values, and pharmacokinetic principles.
  • Prepare, verify, and dispense sterile compounded medications in accordance with USP standards and regulatory requirements.
  • Educate patients, caregivers, and healthcare providers on medication therapy, infusion administration, and medication safety.
  • Collaborate with physicians, nurses, and interdisciplinary care teams to ensure continuity of care and positive patient outcomes.
  • Manage infusion pumps and medication administration equipment while maintaining quality and safety standards.
  • Provide drug information, maintain accurate documentation, and support compliance and medication safety initiatives.

What We're Looking For

  • PharmD or graduate of an accredited School of Pharmacy.
  • Active, unrestricted Texas Pharmacist license.

One or more of the following:

  • At least 1 year of sterile IV compounding experience.
  • Experience with non-sterile compounding, immunizations, and Medication Therapy Management (MTM).
  • Completion of an ASHP-accredited Pharmacy Residency.
  • Completion of an Advanced Pharmacy Practice Experience (APPE).
  • Equivalent pharmacy experience.
  • Strong clinical knowledge of home infusion pharmacy practices.
  • Experience interpreting laboratory values and applying pharmacokinetic principles.
  • Excellent communication, patient counseling, and customer service skills.
  • Strong attention to detail and commitment to patient safety and quality care.

License Required: Active Texas Pharmacist License
