Sterile Compounding Pharmacist
13-Week Contract | Schedule: 12:00 PM - 8:00 PM
We are seeking a Sterile Compounding Pharmacist to support a home infusion pharmacy team. This role combines clinical pharmacy expertise, sterile compounding, and patient-centered care to ensure safe and effective medication therapy for patients receiving treatment at home.
What You'll Do
- Evaluate patients for home infusion therapy and determine eligibility for home-based treatment.
- Develop, monitor, and optimize individualized medication therapy plans using clinical assessments, laboratory values, and pharmacokinetic principles.
- Prepare, verify, and dispense sterile compounded medications in accordance with USP standards and regulatory requirements.
- Educate patients, caregivers, and healthcare providers on medication therapy, infusion administration, and medication safety.
- Collaborate with physicians, nurses, and interdisciplinary care teams to ensure continuity of care and positive patient outcomes.
- Manage infusion pumps and medication administration equipment while maintaining quality and safety standards.
- Provide drug information, maintain accurate documentation, and support compliance and medication safety initiatives.
What We're Looking For
- PharmD or graduate of an accredited School of Pharmacy.
- Active, unrestricted Texas Pharmacist license.
One or more of the following:
- At least 1 year of sterile IV compounding experience.
- Experience with non-sterile compounding, immunizations, and Medication Therapy Management (MTM).
- Completion of an ASHP-accredited Pharmacy Residency.
- Completion of an Advanced Pharmacy Practice Experience (APPE).
- Equivalent pharmacy experience.
- Strong clinical knowledge of home infusion pharmacy practices.
- Experience interpreting laboratory values and applying pharmacokinetic principles.
- Excellent communication, patient counseling, and customer service skills.
- Strong attention to detail and commitment to patient safety and quality care.
License Required: Active Texas Pharmacist License
