This opportunity involves providing clinical pharmacy support in a Long-Term Acute Care (LTAC) hospital located in Marin County, CA. The role offers a flexible part-time schedule with the potential to transition into a full-time position. Key Responsibilities:

  • Review and verify medication orders in an LTAC hospital environment
  • Support clinical pharmacy services including pharmacokinetics and anticoagulation monitoring as needed
  • Assist with medication dispensing and manage automation workflows using Pyxis systems
  • Collaborate with interdisciplinary care teams to optimize patient outcomes
  • Ensure safe, effective medication management tailored to the needs of medically complex patients

Qualifications:

  • Bachelor of Science or Doctor of Pharmacy (PharmD) from an accredited School of Pharmacy
  • Active, unrestricted California Pharmacist License in good standing

Preferred Experience:

  • 2–3 years of retail pharmacy experience with interest in transitioning to a clinical setting
  • Prior inpatient or hospital pharmacy experience
  • Experience with Total Parenteral Nutrition (TPN) preparation and monitoring
  • Knowledge of pharmacokinetics and anticoagulation management
  • Current BLS and ACLS certifications are advantageous
