Key Responsibilities:

• Review and verify medication orders to ensure accuracy, appropriateness, and regulatory compliance.
• Provide clinical oversight for infusion therapies including specialty medications, biologics, antimicrobials, Total Parenteral Nutrition (TPN), and compounded sterile preparations.
• Assess patient-specific therapy plans and monitor treatment outcomes to enhance therapeutic effectiveness.
• Collaborate closely with physicians, nurses, and other healthcare professionals to optimize medication management and therapy recommendations.
• Maintain compliance with state and federal pharmacy regulations, accreditation standards, and organizational policies.

Qualifications:

• Must be local to Seattle, WA.
• Current and active Washington State Pharmacist license.
• Minimum of 2 years clinical pharmacy experience; home infusion experience is highly preferred.
• Proven clinical expertise with inpatient, IV, home infusion, TPN, vancomycin, biologics, and sterile compounding.
• Specialty experience caring for patients with rare diseases or complex medical needs is a strong plus.
