Key Responsibilities:

• Collaborate closely by rounding with the patient care team to provide expert medication interventions.
• Develop and maintain treatment protocols using Epic/Epic Beacon systems.
• Facilitate smooth transitions of care to outpatient oncology pharmacists.
• Manage non-formulary medication requests and coordinate specialty medications for inpatient oncology patients.
• Ensure compliance with clinical standards and optimize pharmacological care for improved patient outcomes.

Qualifications and Experience:

• Valid Texas pharmacist license and Texas IV certification required.
• Minimum of 3 years’ inpatient pharmacy experience, with a strong preference for oncology specialization.
• Proficiency in Epic and Epic Beacon systems is preferred.
• Exceptional communication skills and ability to work effectively in a multidisciplinary team environment.
