Clinical Pharmacist in Sun Prairie, WI

A healthcare facility in Sun Prairie, WI is seeking three Inpatient Pharmacists for a 13-week contract starting June 29. This opportunity offers a range of shifts to accommodate different schedules, with all shifts requiring a commitment to 5 days a week, 8 hours per day, including rotating every third weekend.

Shift Options:

• Day Shift: 6:30 AM – 3:00 PM
• Mid Shift: 11:00 AM – 7:30 PM
• Evening Shift: 2:30 PM – 11:00 PM

Key Responsibilities:

• Provide clinical pharmacy services in an inpatient/acute care setting.
• Perform pharmacokinetic and renal dosing.
• Monitor therapeutic medication effectiveness and safety.
• Manage drug shortages efficiently.
• Deliver pharmacotherapy management addressing various medical problems.
• Utilize EPIC electronic medical records for patient documentation and medication management.

Qualifications:

• Minimum of 2 years recent inpatient or acute care pharmacy experience.
• Proficiency in pharmacokinetic and renal dosing, therapeutic monitoring, and drug shortage handling.
• Experience with EPIC EMR system is required.
• Must possess a Wisconsin pharmacy license; ACT-10 license is acceptable.

Additional Requirements:

• Travelers must be clean-shaven for mask fit testing; use of personal PAPRs or CAPRs is not permitted.
• Scrubs will be provided upon starting the assignment.
