Clinical Pharmacist | Sylmar, California

A large health system with multiple ambulatory care/outpatient clinics and several hospitals seeks experienced clinical pharmacists for a contract opportunity with potential for permanent placement. This role is based in Sylmar, CA, and is ideal for pharmacists with a strong background in inpatient settings.

Responsibilities:

• Provide expert clinical pharmaceutical care across various inpatient and outpatient settings
• Collaborate with healthcare teams to optimize medication therapy and patient outcomes
• Conduct thorough medication reviews and participate in clinical decision-making
• Support quality improvement initiatives and ensure compliance with regulatory standards
• Engage in patient education and counseling as needed

Qualifications:

• PGY1 residency completion is highly preferred
• Approximately 2 years of current inpatient pharmacy experience
• Strong knowledge of clinical pharmacy practices and drug therapies
• Excellent communication and teamwork skills
• Ability to adapt in a fast-paced healthcare environment

Schedule/Hours:

• Full-time shifts within ambulatory and hospital settings
• Flexibility to work on-site for interviews and ongoing assignments
