Ambulatory Care Pharmacist

Pharmacist opportunity focused on Transitions of Care/Ambulatory Care in a large LA County hospital system. 

This is a great fit for a PGY1 pharmacist, but strong non PGYI candidates; including retail/outpatient pharmacists will be considered. Spanish fluency is a plus.

Position Details:

• Schedule: 8-hour shifts; must be flexible for 1st, 2nd, and occasional 3rd shifts
• Duration: Contract to hire; temporary period can last approximately 4-12 months
• Potential: Opportunity to transition into a permanent position within the LA county network

Responsibilities:

• Provide medication education and counseling prior to discharge
• Assist with medication refills before patients leave the hospital
• Support medication adherence and safe transitions home
• Participate in a Meds-to-Beds program
• Provide ambulatory/transition-of-care pharmacy services

Ideal Candidate:

• Active California Pharmacist License
• PGY1 preferred
• Approximately 2+ years of current inpatient experience will be considered
• Retail/outpatient pharmacy experience may be considered
• Strong patient counseling and communication skills
• Spanish fluency is a plus
