Clinical Pharmacist - Syracuse, NY

A specialized clinical pharmacist role is available, focused on home infusion services. This position involves preparing, dispensing, and monitoring complex IV and subcutaneous medications, serving as a critical link within a multidisciplinary healthcare team. The role demands a high level of clinical expertise, particularly in sterile compounding, pharmacokinetic dosing, and patient education.

Key Responsibilities:

• Sterile Compounding: Accurately prepare specialty pharmaceuticals and intravenous solutions in compliance with all relevant state and federal regulations within a sterile, ventilated environment.
• Clinical Monitoring: Evaluate laboratory results, perform pharmacokinetic dosing calculations (e.g., vancomycin), and monitor Total Parenteral Nutrition (TPN) therapy to optimize patient care. Conduct thorough patient counseling about specialty medications and treatment plans.
• Patient Onboarding & Education: Instruct patients and caregivers on infusion pump use, sterile technique maintenance, and catheter care to support effective home therapy.
• Care Coordination: Collaborate with prescribing physicians, home health nurses, and internal teams including intake, referral services, reimbursement, nursing, clinical services, and pharmacy operations to ensure continuity of care and prevent hospital readmissions.
• Regulatory Compliance: Adhere strictly to State Board of Pharmacy regulations, HIPAA, FDA, DEA, Medicare, ACHC, URAC, OSHA, and other applicable standards.
• Inventory & Shipping: Manage inventory control, including refrigerated products, and oversee the packaging and shipping of medications and supplies for secure, timely delivery to patients’ homes.

Qualifications:

• Minimum of 2 years’ experience in infusion specialty pharmacy.
• Experience with IVIg therapies is highly desirable.
• Comprehensive knowledge of specialty therapy payer requirements, reimbursement processes, and contract obligations.
• Current and valid NY pharmacist license in good standing.
• Proficiency with the CareTend system.
