A clinical pharmacist specializing in home infusion plays a vital role in preparing, dispensing, and monitoring complex intravenous (IV) and subcutaneous medications for patients receiving care at home. This position requires close collaboration within a multidisciplinary team to ensure optimal patient outcomes through sterile compounding, medication adjustments, and impactful patient education.

Core Responsibilities:
- Sterile compounding of specialty pharmaceuticals and IV solutions in compliance with state and federal regulations, maintaining a sterile, ventilated environment.
- Clinical monitoring including assessment of lab values, pharmacokinetic dosing (e.g., vancomycin), and Total Parenteral Nutrition (TPN) adjustments. Provide thorough patient counseling on specialty medications and treatment adherence.
- Patient onboarding and education on infusion pump use, sterile technique, and catheter care to promote safe and effective therapy at home.
- Coordination of care alongside prescribing physicians, home health nurses, and internal teams such as intake, reimbursement, and pharmacy operations to optimize therapy management and reduce hospital readmissions.
- Ensuring regulatory compliance with State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, OSHA, and other applicable standards.
- Managing inventory, including refrigerated products, and preparing medication shipments to ensure safe, prompt delivery.
Qualifications:
- Minimum 2 years of infusion specialty pharmacy experience.
- Experience with IVIg therapy is a strong plus.
- Working knowledge of specialty therapy payer requirements, reimbursement processes, and contract obligations.
- Current and active New York pharmacist license in good standing.
- Familiarity with CareTend pharmacy system preferred.
