Clinical Pharmacist in Syracuse, NY

A clinical pharmacist specializing in home infusion is needed for a direct hire opportunity in Syracuse, NY. This role involves preparing, dispensing, and monitoring complex IV and subcutaneous medications while serving as a critical link within a multidisciplinary care team. The position focuses on ensuring safe sterile compounding, optimizing individualized patient therapies, and providing essential education to patients and caregivers.

Key Responsibilities:

• Prepare specialty pharmaceuticals and intravenous solutions in a sterile, ventilated environment compliant with state and federal regulations.
• Monitor lab values, calculate pharmacokinetic dosing (e.g., vancomycin), and manage Total Parenteral Nutrition (TPN) therapies to adjust treatments accordingly.
• Provide detailed counseling on specialty medications and treatment plans to patients and caregivers.
• Educate patients on using infusion pumps, sterile techniques, and catheter maintenance for at-home care.
• Collaborate closely with prescribing physicians, home health nurses, and internal teams including intake, referral services, reimbursement, nursing, clinical services, and pharmacy operations to support coordinated patient care and prevent hospital readmissions.
• Maintain compliance with regulatory standards such as State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, and OSHA.
• Oversee pharmacy inventory management, including refrigerated products, and prepare medication shipments for safe, timely delivery.

Qualifications/Desired Experience:

• Minimum of 2 years’ experience in infusion specialty pharmacy.
• Experience with intravenous immunoglobulin (IVIg) therapies is advantageous.
• In-depth knowledge of specialty therapy payer requirements, reimbursement processes, and contract obligations.
• Active and current NY pharmacist license in good standing.
• Familiarity with CareTend pharmacy management system.
