Infusion/Clinical Pharmacist

We’re seeking an experienced Infusion/Clinical Pharmacist for a direct-hire opportunity with a specialty home infusion pharmacy in Syracuse, NY!

Details:

• Schedule: Monday–Friday, 8:00 AM–4:30 PM (potential on-call rotation) 
• Start: ASAP
• Type: Direct Hire/Permanent 
• System: CareTend

Responsibilities:

• Prepare and dispense IV and specialty medications using sterile compounding techniques
• Monitor labs, pharmacokinetic dosing, TPN, and medication therapies
• Provide patient and caregiver education on infusion therapy and home administration
• Collaborate with physicians, nurses, and pharmacy teams to coordinate patient care
• Ensure compliance with applicable pharmacy, healthcare, and regulatory standards
• Manage inventory and coordinate medication and supply shipments

Requirements:

• 2+ years of infusion specialty pharmacy experience
• Current, active NY Pharmacist license in good standing
• Experience with specialty therapy payer requirements and reimbursement processes
• IVIg experience is a plus
