Infusion Pharmacist

HOT opportunity for an AZ-licensed pharmacist!

We're looking for a Staff Clinical/Infusion Pharmacist for an 8-12 week contract with a home infusion pharmacy in the Phoenix area. This is a great fit for a pharmacist with IV room, hospital, or infusion pharmacy experience.

Position Details:

• 8-12 week contract
• Start ASAP
• Monday-Friday, 8:30 AM-5:00 PM
• May be asked to participate in an on-call rotation
• AZ pharmacist license required
• System: CareTend

Responsibilities:

• Prepare and dispense complex IV and subcutaneous medications
• Perform sterile compounding in accordance with applicable regulations
• Monitor lab values and pharmacokinetic dosing, including vancomycin
• Monitor and adjust TPN therapy
• Provide patient and caregiver education on specialty medications and home infusion therapy
• Monitor drug therapies and recommend clinical interventions
• Coordinate care with physicians, nurses, intake, reimbursement, and pharmacy operations teams
• Maintain compliance with applicable pharmacy and healthcare regulations
• Manage pharmacy inventory and prepare medications and supplies for delivery

Requirements:

• Active, current AZ pharmacist license in good standing
• 1-3 years of infusion specialty pharmacy or hospital pharmacy experience
• Hospital candidates should have primary experience working in the pharmacy IV room
• IVIg experience is a plus
• Knowledge of specialty therapy payer requirements, reimbursement processes, and contract obligations preferred
