Per Diem Clinical Pharmacist | Turlock, California

A part-time clinical pharmacist opportunity is available to support infusion services in a well-established outpatient setting. This role provides 2-3 days per week of work Monday through Friday from 8:30 a.m. to 5:00 p.m., ideal for candidates based near Modesto, Stockton, or Merced seeking flexible per diem hours.

Key Responsibilities:

• Review and evaluate physician prescription orders for dosage accuracy, drug choice appropriateness, interactions, administration routes, and stability.
• Maintain comprehensive patient records, including current height, weight, medical history, lab results, and concurrent medications.
• Ensure compliance with regulatory documentation requirements from state, federal, and accrediting organizations.
• Order and review clinical laboratory tests to assess medication safety and efficacy in collaboration with nurses and physicians.
• Provide clinical and technical support to physicians, nurses, and patients.
• Assist in implementing pharmacy policies and oversee the training and supervision of pharmacy staff.
• Supervise sterile compounding operations and quality assurance programs, including validation and testing procedures.
• Document all professional communications and ensure patient safety through excellent pharmacy practices.
• Educate patients and caregivers on drug therapies and medication management.

Qualifications:

• Valid California pharmacy license in good standing.
• Strong background in intravenous (IV), infusion, and clinical pharmacy.
• Prior experience in home infusion pharmacy strongly preferred.
• Familiarity with CareTend pharmacy system is an advantage.
