The position is primarily clinical, focusing on Population Health and Ambulatory Care, dedicating approximately 60% of the time to managing medication regimens for patients, especially those with targeted disease states such as diabetes. The remaining 40% involves staffing duties, supporting the pharmacy operations in a hybrid capacity. As the clinical program grows, there is potential to transition into a fully clinical role.

Responsibilities

  • Manage patient medication regimens, focusing on disease state management (e.g., diabetes)
  • Collaborate with healthcare teams to optimize ambulatory care services
  • Serve in both clinical and staffing roles, balancing patient care with pharmacy operations
  • Utilize Meditech IT system for documentation and communication
  • Contribute to the growth and expansion of clinical pharmacy services

Qualifications & Desired Experience

  • Licensed Pharmacist in Pennsylvania
  • Experience in ambulatory care or inpatient pharmacy preferred
  • Residency training considered a significant advantage
  • Strong clinical skills with a passion for patient-centered care
  • Ability to work variable days and afternoon shifts, primarily 8-hour day shifts

Benefits & Compensation

  • Competitive salary range between $106,000 and $165,000 annually (mid-point around $135,000)
