Responsibilities:

• Deliver clinical pharmacy services focused on Population Health and Ambulatory Care, with an emphasis on managing medication regimens for targeted disease states, such as diabetes.
• Provide staffing support approximately 40% of the time to ensure smooth pharmacy operations.
• Collaborate with multidisciplinary teams to optimize patient outcomes within the health system.
• Contribute to the growth and potential expansion of clinical pharmacy programs, transitioning towards a fully clinical role as opportunities develop.

Desired Experience and Qualifications:

• Licensed Pharmacist in Pennsylvania.
• Experience in ambulatory care or inpatient pharmacy practice.
• Post-graduate residency training strongly preferred.
• Strong patient care orientation and proactive approach to clinical program development.
• Familiarity with Meditech electronic health record systems is an advantage.
