• Review and verify medication orders for accuracy and appropriateness
• Collaborate with healthcare teams to optimize patient medication therapy
• Monitor patient responses and adjust treatments as necessary
• Utilize clinical IT systems such as Cerner and Pyxis for medication management
• Support compliance with hospital policies and regulatory requirements

Qualifications:

• Must hold a Pharmacist License valid in Oregon, preferably with OR Pharmacist License
• Some experience working in inpatient clinical pharmacy settings
• Proficient in clinical IT systems, specifically Cerner and Pyxis
• Strong communication and problem-solving skills
• Ability to work independently and as part of a team
