Clinical Pharmacist | Waukesha, Wisconsin

This opportunity is ideal for experienced inpatient pharmacists with a solid background in clinical pharmacy looking to contribute in a dynamic hospital setting. The role is based in Waukesha, WI, serving a reputable hospital in Madison, WI, on a contract basis for 13 weeks, starting June 8.

Schedule & Hours:

• 5 days per week, 8-hour shifts
• Rotating shifts between days (6:30 AM – 3:00 PM) and evenings (2:30 PM – 11:00 PM)
• Rotating weekend coverage every third weekend

Key Responsibilities:

• Provide clinical pharmacy services within an inpatient setting
• Perform pharmacokinetic and renal dosing adjustments
• Monitor therapeutic medication outcomes
• Manage drug shortages effectively
• Deliver pharmacotherapy management tailored to various medical conditions
• Utilize EPIC electronic medical records for documentation and medication management

Qualifications:

• Minimum of 2 years recent inpatient or acute care pharmacy experience
• Valid Wisconsin pharmacy license; ACT-10 license holders are eligible
• Strong skills in pharmacokinetic and renal dosing
• Experience with therapeutic drug monitoring and pharmacotherapy management

Additional Requirements:

• Travelers must be clean-shaven for mask fit testing, as CAPR and PAPR devices are not permitted or provided
• Non-compliance with fit testing requirements will result in immediate cancellation of the assignment
