Clinical Pharmacist | Westerville, Ohio

An opportunity is available for experienced IV pharmacists at a growing closed-door Long Term Care pharmacy specializing in complex sterile compounding and clinical monitoring. This direct hire role is based in Westerville, OH, supporting advanced medication therapies for patients with chronic diseases.

Two positions are open:

• IV Lead Pharmacist (1st shift): Flexible Monday–Friday schedule, either 7:00 am to 3:30 pm or 8:00 am to 4:30 pm
• IV Staff Pharmacist (2nd shift): Flexible Monday–Friday schedule, either 4:30 pm to 12:00 am or 6:00 pm to 2:30 am

License: Must hold an active Ohio pharmacist license

Key Responsibilities:

• Prepare, check, and dispense intravenous (IV) medications, including hazardous drugs, TPN, and biologics, following USP 797/800 and company protocols
• Evaluate clinical appropriateness of drug orders, monitor patient responses, and make therapy adjustments for complex conditions such as oncology, immunology, and transplant
• Educate patients on medication administration techniques and potential side effects to optimize therapy outcomes
• Ensure regulatory compliance through detailed documentation, cleanroom maintenance, and adherence to state and federal laws
• Collaborate with healthcare providers including nurses, physicians, and case managers to coordinate care and ensure timely delivery of medications

Desired Experience:

• Proven expertise in sterile compounding and handling of intravenous therapies
• Strong clinical knowledge and the ability to manage high-cost, specialized medication regimens
• Familiarity with USP 797/800 compliance and quality assurance standards
