A professional clinical pharmacist role is available in Westerville, OH, focusing on home infusion services. This direct hire position involves managing complex intravenous (IV) and subcutaneous medication therapies while serving as the pivotal point for a multidisciplinary care team.

Key Responsibilities:

• Sterile Compounding: Accurately prepare specialty pharmaceuticals and IV solutions in a sterile, ventilated environment, adhering to all state and federal regulations.
• Clinical Monitoring: Evaluate lab values, perform pharmacokinetic dosing calculations (e.g., vancomycin), monitor Total Parenteral Nutrition (TPN), and adjust medication regimens accordingly. Provide detailed patient counseling on specialty medications and treatment plans.
• Patient Onboarding & Education: Instruct patients and caregivers on the proper use of infusion pumps, sterile techniques, and catheter maintenance in the home setting.
• Care Coordination: Work collaboratively with prescribing physicians, home health nurses, and internal teams—including intake, reimbursement, nursing, clinical services, and pharmacy operations—to ensure efficient therapy management and reduce hospital readmissions.
• Regulatory Compliance: Ensure all activities comply with State Board of Pharmacy regulations, HIPAA, FDA, DEA, Medicare, ACHC, URAC, OSHA, and other relevant standards.
• Inventory & Shipping: Oversee pharmacy inventory management, including refrigerated products, and prepare medication shipments to guarantee safe, timely delivery to patients.

Desired Experience:

• Minimum of 2 years’ experience in infusion specialty pharmacy
• IVIg experience is highly valued
• Familiarity with specialty therapy payer requirements, reimbursement processes, and contract obligations
• Proficiency with the CareTend system
• Current Ohio pharmacist license in good standing
