Responsibilities:

• Provide clinical pharmacy services to support inpatient care

• Oversee warfarin therapy management

• Utilize electronic medical records, primarily EPIC system

Qualifications:

• Active Ohio Pharmacist license in good standing
• Minimum one year of experience in inpatient hospital pharmacy
• Proficiency in pharmacokinetic dosing and warfarin management
• Experience with EPIC Willow Inpatient is preferred, but not required
• Flexibility to work varied shifts including evenings and weekends
