Pharmacist - Arlington, TN

An opportunity is available for an experienced Infusion/Clinical Pharmacist to support a multidisciplinary team in managing complex intravenous (IV) and subcutaneous medication therapies at a site in Tennessee.

This is a direct hire position with an immediate start date. The work schedule is Monday through Friday, 8:00 a.m. to 5:00 p.m., including participation in an on-call rotation (typically fewer than five after-hours calls per week).

Responsibilities:

• Prepare sterile specialty pharmaceuticals and IV solutions in compliance with state and federal regulations.
• Monitor lab values, perform pharmacokinetic dosing adjustments (e.g., vancomycin), and manage Total Parenteral Nutrition (TPN).
• Provide patient counseling and education on specialty medications, infusion pump use, sterile techniques, and catheter maintenance.
• Coordinate care with physicians, home health nurses, and interdisciplinary teams to ensure effective therapy management and reduce hospital readmissions.
• Ensure compliance with relevant regulations including State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, and OSHA.
• Manage inventory, including refrigerated products, and oversee safe packaging and timely medication delivery.

Qualifications:

• Minimum of 3 years’ infusion specialty pharmacy experience.
• IVIg and hemophilia experience preferred.
• Knowledge of specialty therapy payer requirements, reimbursement processes, and contract obligations.
• Current and good standing pharmacist license in Tennessee.
• Experience with CareTend system is a plus.
