A travel pharmacist opportunity is available in Vermont at a 100-bed community hospital within a respected health network. This role offers a chance to work with a dynamic team while providing essential pharmacy services at a well-established facility.

Location:

  • Barre, Vermont (with some functions in Berlin)
  • Part of a prestigious university-affiliated health system

Schedule:

  • Monday through Friday
  • Shifts available: 6:30 AM–3:00 PM, 10:00 AM–6:30 PM, or Noon–8:00 PM

Duration:

  • 4 to 6 months
  • ASAP start date

Responsibilities:

  • Perform medication reconciliation to ensure accuracy and safety
  • Verify medication orders in a timely and precise manner
  • Manage vancomycin dosing and laboratory monitoring
  • Monitor and adjust renal dosing as needed
  • Collaborate with healthcare teams to optimize patient outcomes

Qualifications:

  • Minimum of 5 years of pharmacy experience with strong intravenous (IV) medication expertise
  • Hold a Vermont license or qualify for a provisional temporary license, with willingness to obtain a permanent license during assignment
  • Demonstrated ability to manage complex dosing regimens and laboratory-related adjustments
