Key Responsibilities:

• Perform pharmacist duties in a long-term care specialty pharmacy setting
• Manage medication therapy and provide patient counseling
• Coordinate with healthcare providers to ensure optimal patient outcomes
• Ensure compliance with state and federal regulations
• Oversee medication distribution and maintain accurate records
• Provide immunizations as authorized and participate in health screening programs

Qualifications and Desired Experience:

• Licensed pharmacist with active licensure in the state
• PharmD preferred
• Immunization Certification required
• CPR Certification required
• Experience or certification in Medication Therapy Management (MTM) a plus
• Strong knowledge of long-term care pharmacy practices

Schedule:

• Monday through Friday, 8:30 AM to 5:30 PM
• Some weekend shifts required
