A critical pharmacist position is available for a 20-week travel assignment starting March 17th in Carbondale, CO, supporting an EMR transition from Meditech to Epic. This role demands expertise in pharmacy operations, exceptional clinical judgment, and strong communication skills within a dynamic healthcare environment.
Responsibilities:
- Prepare and dispense medications accurately per physician orders.
- Maintain medication inventories in patient care areas.
- Ensure safe, cost-effective drug therapies aligned with policies and protocols.
- Interpret medication orders and update computerized patient medication profiles.
- Compound pharmaceuticals, including IV admixtures, chemotherapy, and parenteral nutrition.
- Manage controlled substances with required documentation.
- Monitor drug regimens for interactions, allergies, and dosages.
- Perform pharmacokinetic dosing and review antibiotic appropriateness.
- Provide clinical consultation, drug information, and therapeutic alternatives to healthcare practitioners.
- Document clinical activities and participate in quality improvement and training programs.
- Serve as a member of the Code Response Team.
Qualifications:
- Active Colorado Pharmacist License in good standing.
- Graduated from an ACPE-accredited School of Pharmacy with a BS Pharmacy degree.
- Valid BLS and ACLS certifications from the American Heart Association.
- Flu vaccine compliance mandatory (no exemptions; required October 1 through April 1).
- MUST have Epic experience; Meditech training is a plus.
Schedule:
- Rotating day and evening shifts, 40 hours/week.
- Start times at 6:30 AM or 8:00 AM Monday through Friday.
- Rotating weekends 7:00 AM to 12:00 PM.
